Background-check Disclosure & Authorization

This disclosure and authorization applies to counselors and is referenced by the Counselor Professional Terms. You provide your authorization by affirmative acceptance (e-signature) during counselor signup.
Part A — Disclosure
Admito, through a consumer reporting agency (our background-check provider, Checkr, Inc.), may obtain a consumer report and/or investigative consumer report about you for the purpose of evaluating your eligibility to serve as a counselor on the platform, including child-safety screening. This disclosure is provided to you in a document that consists solely of the disclosure, as required by the federal Fair Credit Reporting Act (FCRA).
Part B — Authorization
I authorize Admito LLC and its background-check provider to obtain the report(s) described above, now and (where I remain an active counselor and permitted by law) on a recurring basis. I understand I may request information about the nature and scope of the report and a copy of my rights under the FCRA ("A Summary of Your Rights Under the FCRA").
Admito initiates and manages this screening through its chosen provider; a background check you obtain yourself is not accepted in its place. You agree to pay Admito's verification fee for this screening, as shown at signup. You provide your signature by affirmative acceptance (e-signature) at signup, with the date recorded.
Part C — Identity verification
Separately, Admito uses an identity-verification provider (Stripe Identity) to verify your identity. This is distinct from the background check.
Part D — Adverse-action process
If information in a report may lead Admito to deny, suspend, or remove you:
- Pre-adverse-action notice — Admito will send you notice, a copy of the report, and the FCRA summary of rights, and allow a reasonable period (at least 5 business days) to dispute or explain.
- Adverse-action notice — if Admito proceeds, it will send the final notice, including the provider's name and contact, a statement that the provider did not make the decision, and your right to a free report copy and to dispute its accuracy.
Where state or local law imposes additional notice, timing, or "ban-the-box" requirements, Admito applies the more protective standard and provides the FCRA summary of rights together with any required state addenda.
